How to Qualify

I had the choice of any home health care agency that I wanted. There are so many, but I chose Premier Home Health Providers! You have the choice too!!To qualify for the service, the patient must be under the care of a physician, dentist or podiatrist, must be homebound and in need of intermittent skilled care. This may include instructions or hands on care as ordered by their primary physician.

If the following applies you may be Homebound and eligible for Home Health Care from the provider of your choice:
  • Leaving your home isn't recommended because of your condition
  • Your condition keeps you from leaving home without help (such as using a wheelchair or walker, needing special transportation, or getting help from another person)
  • Leaving home takes a considerable and taxing effort
  • A person may leave home for medical treatment or short, infrequent absences for non-medical reasons, such as attending religious services. You can still get home health care if you attend adult day care, but you would get the home care services in your home.

Send your referral to Premier Home Health. We accept referrals from a physician, they may also originate from any local hospital, doctor's office, board and care facility, patient, family, interested person or agency. A nurse or qualified representative from Premier will receive referral information. The patient is signed up for the various services the physician requests. Within 24-48 hours, our representative will see the patient, unless specific care is required on the same day.
